I called Rutgers they said they would forward me the questions however they never sent anything.. It appears to be another controlled report that will be used by the leftist liberals to gather and justify info for their totalitarian
Now my wife swears the parents sign a consent form when a child is born or at doctors office getting shots when children are young, can you provide any proof that this is not happening with out parents consent also couple doctors have said parents sign consent forms ?
The state says it's "automatic enrollment " it's in their documentation when the child gets a birth certificate. That implies that you didn't sign anything.

I just filled out the form, but there is a field for a "Medical Record Number" and I have no clue what that is. Does anyone know?

You are signing that you understand the risks of the vaccine in the drs office. You are not signing to be inrolled in a database
