It is hard to believe this is legal. It is another gross overstep of the government. I am immediately removing my kids from this disgrace.
I would also like to publish this information on our website and wherever I am able to get the information out. Privacy must be protected at all costs - especially among those who cannot advocate for themselves, our children.

I called Rutgers they said they would forward me the questions however they never sent anything.. It appears to be another controlled report that will be used by the leftist liberals to gather and justify info for their totalitarian
Now my wife swears the parents sign a consent form when a child is born or at doctors office getting shots when children are young, can you provide any proof that this is not happening with out parents consent also couple doctors have said parents sign consent forms ?

I just filled out the form, but there is a field for a "Medical Record Number" and I have no clue what that is. Does anyone know?
